Important Instructions

  • Starting from 01-04-2024, Registration can be done up to two weeks after declaration of 12th results. (Registration process will remain open after 15-04-2024)

  • Applicant must have an active Email Id and Mobile Number

  • Applicant can register only once, from the same email id.

  • Applicant must have latest Photo and Signature with maximum size of 50 K.B.

  • Applicant must have below documents with maximum size of 200 K.B. for complete application fill-up.

For Physiotherapy Candidates:

1. Registration Slip generated Online
2. 12th Marksheet
3. All BPT Mark sheets of all years (Both Pass & Fail Marksheets)
4. Attempt Certificate
5. Internship Completion Certificate by College or University
Or
Certificate from Dean/Principal regarding tentative date of Completion of internship.
6. Caste certificate (If Applicable) – For SEBC/SC/ST Candidates
7. Non Creamy layer Certificate issued after 01/04/2021 by the competent authority as
Prescribed by the Govt. of Gujarat – For SEBC Candidates
8. EWS (Economical Weaker Section) certificate issued after 01/04/2023 by competent
Authority as prescribed by the Govt. of Gujarat – For EWS Candidates
9. School leaving certificate or Birth Certificate or Proof of Birth Date and Birth Place
10. For Employed Candidates- Study leave/Resignation/NOC (If Applicable)
11. For PwD (Person with Disability) candidates – Disability Certificate by Competent Authority

For Optometry Candidates:

1. Registration Slip generated Online
2. 12th Marksheet
3. All B. Optometry Mark sheets of all years (Both Pass & Fail Marksheets)
4. Attempt Certificate

Superintendent/Nursing Superintendent/Registrar of Hospital
6. Degree/Provisional Degree Certificate
7. Gujarat Nursing Council/Respective State Nursing Council Registration Certificate
8. Caste Certificate – For SEBC/SC/ST Candidates
9. Non Creamy layer Certificate issued after 01/04/2021 by the competent authority as
Prescribed by the Govt. of Gujarat – For SEBC Candidates
