3. Photo copy of Copy of Six Months/One Year Housemanship/Internship Completion Certificate issued by Gujarat University (For Student of M.B.B.S., B.D.S., B.P.O., B.O.T., B.Opto., B.H.M.S. Bachelor of Physiotherapy)
4. Photo copy of Six Months/One Year practical training Certificate (For Student of B.Pharm/D.Pharm).
5. Please read form carefully before submission.
6. The prescribed fee of Rs. 252/- is paid online.
7. The Certificate Will Be Valid only For Six Months From Date of Issue.
8. Certificate will be ready by 15 working days. Collect Your Certificate Within One Month From Date of Submission of Document in University. Collect your Certificate with one month from date of Submission of Document in University.
9. Students who have passed exam in April/May 1991 cannot apply for Provisional Degree Certificate. Such candidate should contact Convocation branch, Exam Section Room number 42 with Final Year Marksheet.
10. All the regular students who have passed exams of BA/B.Com/B.Sc/BBA/BCA/B.Ed/ MA/M.Com/M.Sc between April/May 2011 to Nov/Dec 2017 cannot apply for Provisional Degree Certificate. Such candidate should contact Convocation branch, Exam Section Room number 42 with Final Year Marksheet.
11. If Degree is conferred as per our records then fees paid will not be refunded.

12. કામચાલાઉ પદવી પ્રમાણપત્ર (Provisional Degree Certificate) મેળવવા માટે ફોર્મ તથા ફી ઓનલાઈન ભરવાની રહેશે. ઓનલાઈન ફોર્મ તથા ફી ભર્યા બાદ ફોર્મ, ફી ની પહોંચની (Fee Receipt) કોપી તથા છેલ્લા વર્ષની/છેલ્લા સેમેસ્ટરના ગુણપત્રકની (Mark Sheet) નકલ લઇ ગુજરાત યુનિવર્સિટી, રૂમ નંબર – ૪૨, ૧-કોન્વોકેશન વિભાગમાં જમા કરાવવાની રહેશે.

13. Bachelor of Medicine & Bachelor of Surgery, Bachelor of Dental Surgery, Bachelor of Physiotherapy, Bachelor of Optometry, Bachelor of Occupational Therapy, Bachelor of Prosthetics & Orthotics, Bachelor of Homeopathic & Medicine Surgeryના વિદ્યાર્થીઓએ તેમની પદવી વર્ષના ગુણપત્રકની સુવાચ્ય નકલ તથા ગુજરાત યુનિવર્સિટી દ્વારા આપવામાં ઈન્ટર્નશીપ કંમ્પ્લીશન સર્ટિફિકેટની સુવાચ્ય નકલ ગુજરાત યુનિવર્સિટી માં ફોર્મ તથા ફી ની પંહોચ સાથે ગુજરાત યુનિવર્સિટીમાં રૂબરૂ જમા કરાવવાનું રહેશે. Bachelor of Pharmacy/Diploma in Pharmacy ના ગુજરાત યુનિવર્સિટીના વિદ્યાર્થીઓએ કોલેજ દ્વારા આપવામાં આવેલ પાસીંગ સર્ટિફિકેટ/પ્રેક્ટિકલ ટ્રેઈનીંગ સર્ટિફિકેટની સુવાચ્ય નકલ ગુજરાત યુનિવર્સિટી માં ફોર્મ તથા ફી ની પંહોચ સાથે ગુજરાત યુનિવર્સિટીમાં રૂબરૂ જમા કરાવવાનું રહેશે. ગુજરાત યુનિવર્સિટીમાં ફોર્મ જમા કરાવવાની જવાબદારી વિદ્યાર્થીની રહેશે. જો વિદ્યાર્થી ફોર્મ જમા નહી કરાવે ત્યાં સુધી કામચાલાઉ પદવી પ્રમાણપત્ર (Provisional Degree Certificate) બનાવી શકાશે નહિ.

14. વિદ્યાર્થીઓએ ઓનલાઈન ફોર્મ તથા ફી ભર્યા બાદ વધુમાં વધુ ૭ દિવસમાં ફોર્મ તથા ફી તથા અન્ય જરૂરી દસ્તાવેજ જમા કરાવશે તે દિવસથી ચાલુ ૧૫ દિવસ (Working 15 Days) બાદ કામચલાઉ પદવી પ્રમાણપત્ર રૂબરૂ આવીને મેળવવાનું રહેશે. ફોર્મ જમા કરાવ્યા બાદ તુરત કામ ચલાઉ પદવી પ્રમાણપત્ર મેળવવાનો આગ્રહ રાખવો નહિ
